Office of the syndic
Any individual may ask the office of the syndic to conduct a disciplinary investigation if he/she believes that an LPN has committed an infraction.
If you would like the office of the syndic to conduct an investigation, you must submit a written letter briefly describing the nature of the infraction. Be sure to include the following information:
Date of the event.
Name of the LPN.
Address of the institution.
Name and contact information of any other persons involved
- Your first name, last name, address and telephone numbers
Please be sure to sign the letter. You should also attach a copy of any relevant documents.

Professional inspection
The Professional Inspection Committee (PIC) conducts an inspection focusing on the professional competence of an OIIAQ member if it is called into question. Professional competence inspections are designed to evaluate the quality of LPNs’ professional practice and are conducted as part of a confidential process involving the PIC and the OIIAQ member in question.
Following a professional competence inspection, the PIC may make a recommendation to the OIIAQ’s Executive Committee requiring an OIIAQ member to successfully complete:
- A practicum.
- A professional development course.
- Or both.
The PIC may also make a recommendation to the Executive Committee under which the member in question’s right to carry out some or all of his/her professional activities would be limited or suspended until such time as the member has met the requirements or conditions imposed.
In this regard, LPNs as an opportunity to submit observations to the PIC and to the Executive Committee.
